Asthma Collaborative

Orange Primary Care Center, a division of the Newark
Community Health Centers, Inc. (NCHC) Network celebrated
their participation in a New Jersey Statewide Asthma Collaborative,
an initiative to achieve strategic system changes in the
delivery of primary health care services. The collaborative
focuses on the health provider’s team approach to partner
with parents, children and the community to identify and
manage children with asthma. The Asthma Extravaganza
catered to the health center’s current patients and
the presentation was focused on what is asthma, self management,
treatment and triggers. “Our main goal is to make sure
all of our patients are healthy and happy” said Joanne
Williams, Director of Nursing.
East Orange Primary
Care Center partnered with AMERIGROUP NJ to officially
introduce the Asthma Collaborative to the Community. “We
are the safety net for many people in East Orange and we
continue to find ways to strengthen the relationship by offering
the best in primary and preventive care” said Peter
R. Velez, MPH the new president
and chief executive officer of Newark Community Health Centers,
Inc.
The New Jersey Primary Care Association,
(NJPCA), a non-profit corporation that represents the organizational
providers and affiliates of community-based ambulatory health
care statewide, received funding from the New Jersey Department
of Health and Senior Services to initiate a NJ Asthma Collaborative
based on the national model of HRSA’s Health Disparities.
East Orange Primary Care Center is one of the sixteen New
Jersey Community Health Centers that is currently participating
in the Asthma Collaborative.
The mission of the New Jersey Statewide Asthma Collaborative
is to (1) generate and document improved health outcomes
for underserved populations within the state; (2) transform
clinical practices through new and proven effective, evidence-based
models of care; (3) develop infrastructure, expertise and
multi-disciplinary leadership within the sate and within
each organization, to improve health status and; (4) build
strategic partnerships nationally, throughout the state and
with communities.
East Orange Primary
Care Center’s Asthma Collaborative
team includes, Joanne Williams, Director of Nursing, Dr.
Edwina Verner, Director of Pediatrics, Dr. Maxine Facey,
Quality Assurance Coordinator, Claire Roudette, Community
Promise Program Manager, Xavier Wilson, Nutritionist, Kimberly
Jamison, Director of Marketing, William Allen, Grant Writer/Developer,
Esney Sharpe, Executive Assistant and Patricia Tate, Medical
Secretary.
The East Orange Primary Care Center, one
of the 21 Federally Qualified Health Centers (FQHCs) in New
Jersey provides pediatric, OB/GYN, internal medicine, podiatry,
optometry and dental care, as well as lab and social services. For
