S.I.S.T.A.
A group-level intervention for HIV positive
and negative African-American women aimed at reducing HIV
sexual risk behaviors. It is made up of five, two-hour sessions
delivered by peer facilitators in a community-based setting.
The sessions are gender-and culturally relevant and includes
behavioral skill practices, group discussions, role-plays,
and a prevention video and take home exercises. For more

Community PROMISE
PROMISE is an acronym for “Peers Reaching Out and
Modeling Intervention Strategies.” This program is
an effective community-level STD/HIV prevention intervention
that relies on role model stories and peers from the target
population of each community. Community PROMISE is successful
because it is created anew each time it is implemented in
collaboration with a specific community. For more information

HealthStart
This program provides Comprehensive Maternity Care services
for uninsured women. The services provided through this
program consist of, prenatal care and post-partum care,
case coordination, health education, nutrition and social
and psychological services and home visits. For more information

Post Partum Service Coordination
and
Education Programs (PEP)
PEP is a collaboration program between Newark
Community Health Centers, Inc and Health Mothers Healthy
Babies. This program is designed to support all women in
need of pre-natal and post-partum care. The program
focuses on healthy living for the expectant mother and child
before and after pregnancy. For more information, please

340 B Drug Program
Newark Community Health Centers, Inc. participates in the federal 340
B Drug Pricing Program. This program allows NCHC patients
to benefit from discounted medications at designated local
pharmacies near NCHC health center sites
Refugee Health Program (RHP)
Newark Community Health Centers, Inc. is a referral network
site for the Refugee Health Program funded by New Jersey
Department of Health and Senior Services. NCHC provides
comprehensive physical exams and medical treatment to refugee,
asylee and parole arrivals referred by the Refugee Health

Ryan White Title I & III
Newark Community Health Centers, Inc.
provides comprehensive health services and case management
services for HIV positive individuals who are insured and
uninsured. Services include medical, dental, referrals
for substance abuse and mental health, early intervention
for testing and counseling, risk-reduction counseling on
prevention, nutritional and other care services for HIV-infected
clients outreach. Case management is provided
to ensure access to health services and continuity of care. |
